Requirements

• Only one reinstatement is allowed in the lifetime of a credential.
• You must submit the appeal form below to initiate the reinstatement process.
• Fees for Appeals filed between 1-36 months post expiration date (by December 31 of year 3 post expiration) include:
  •  $95 Reinstatement Appeal Fee
    • The payment page will appear immediately after submitting the appeal form below
  • Reinstatement Application Fee 
    • Payment will be due when submitting the reinstatement application

Once your appeal is approved:

• You will receive access to the application portal to complete and submit the reinstatement application
• Your application must be submitted within 30 days of your appeal approval
• You must submit 45 verifiable contact hours of recertification activity earned within the 3 years immediately prior to submitting the reinstatement application and must complete the entire application, including the audit portion.
•  You will be notified of approval of reinstatement within 45 days of submitting your recertification application.
• Your certification period will be back dated to January 1 of the year when your reinstatement is granted.
